Formulas to follow depending upon your situation
Depending upon your situation, you will find below different formulas to follow in order to obtain reimbursement of your medical expenses.
You are in France on holiday or on a short stay*, you can ask for reimbursement of any unforeseeable.
Necessary medical expenses, on presentation of the following documents :
- your European Health Insurance Card (EHIC) ;
- your passport;
- "feuille de soins" (form given by doctor / nurse / dentist / hospital, etc. for forwarding to the Social Security), medical prescriptions and invoices relating to payments made during your care to the nearest caisse d'assurance maladie in the area where you are staying ;
- RIB or IBAN.
Depending upon the nature of your care, you, as the patient will be charged for a contribution to the costs. This varies from 85 % to 20 %.
For example, if you pay a general practitioner 25 €, your contribution will be 8.50 € (7.50 € + 1 € for solidarity).
In the case of hospitalization or of external care, you must present your European health card to the establishment involved. You will be asked to pay your contribution (20%) or any daily hospital charges (meals, telephone, tv, etc.).
* if you are residing in France on a permanent basis, you must refer to the other formulae listed below, and not continue to use your European Health Insurance Card.
Your principal residence is in France, but you are working, or benefit from rights or pension from a European country.
In this case, please, provide us with form E106 (S1) in duplicate.
You can claim it to the National Health System of your country.
On receipt of form E106, please, provide us with :
- the printed Form application for the right to health insurance (S1106) (PDF) - obtainable from the CPAM ;
- birth certificates for you and your family (wife, husband, children, etc.) ;
- marriage certificate ;
- passport or ID copies ;
- proof of residency (Utility Bills, rental contract, etc.) ;
- particulars of your bank account (RIB - relevé d'identité bancaire). These can be found readily available in your French cheque book.
These documents permit us to affiliate you to our system and to take charge of your medical expenses. This follows the French legislation concerning reciprocity with your social security system, and for a period of time as indicated on the form E106 (S1).
Your principal residence is in France, and you receive an invalidity pension (incapacity benefit) or retirement pension from your country.
Please, provide us with a copy of form E 121 (S1), in duplicate, for you, and for each member of your family.
You must also give us :
- the printed Form application for the right to health insurance (S1106) (PDF) ;
- birth certificates for yourself and all members of your family ;
- marriage certificate ;
- passport or ID copies ;
- proof of residency (Utility Bills, rental contract,etc.) ;
- particulars of your bank account (RIB - relevé d'identité bancaire). These can be found readily available in your French cheque book.
These documents permit us to affiliate you to our system and to take charge of your medical expenses according to French legislation.
Following your affiliation, you will be issued a carte Vitale for you and each member of your family above the age of 16.
You are in salaried employment in France ? Your employer must give a declaration of your employment to the Urssaf (Union de recouvrement des cotisations de sécurité sociale et d’allocations familiales).
Following this declaration, please, provide us with the following documents, before proceeding with your affiliation :
- the printed Form application for the right to health insurance (S1106) (PDF) ;
- copy of your work contract or your last pay slips ;
- birth certificates for yourself and all members of your family ;
- marriage certificate ;
- passport or ID copies ;
- proof of residency (Utility Bills, rental contract, etc.) ;
- particulars of your bank account (RIB - relevé d'identité bancaire). These can be found readily available in your French cheque book ;
- your first pay slip.
Once the above requirements are fulfilled, you will be affiliated, and your medical expenses taken in charge, by the caisse primaire d'assurance maladie (CPAM). Its rules apply to you. Once this is in place, you will benefit from payment in kind, reimbursement, etc.
You are living in France for less than 5 years, you do not have a paid activity, you are under retirement age and you do not benefit from any rights from a European country.
You can apply for a PUMa health cover after 3 months of residency in France. If your claim is accepted, you will have to pay a contribution to the Urssaf (Union de recouvrement des cotisations de sécurité sociale et d’allocations familiales). If it's rejected, you will have to find and pay a private insurance.
If you are not eligible to get a S1 form, you can't claim for the PUMa's benefits under 5 years of permanent residence unless your situation has changed while you've been living in France (change of income, divorce, long term disease, etc.).

You are a European citizen, and you are registered at a university or higher education establishment in France. You need to submit to us your European Health Insurance Card (EHIC) covering the period of your studies in France. In this case, you are excused from joining the French student system.
You must also provide us with :
- printed Form application for the right to health insurance (S1106) (PDF) ;
- school or university certificate (student card, certificate, etc.) ;
- your birth certificate ;
- copy of your passport or ID ;
- proof of residency (Utility Bill, rental contract, etc.) ;
- particulars of your bank account (RIB - relevé d'identité bancaire). These can be found readily available in your French cheque book.
These documents permit you to be affiliated to the caisse primaire d'assurance maladie in the area where you will be studying, and allows your medical expenses to be taken in charge sickness, maternity, etc.
If you are older than 28 years old and you are not salaried, you have to be affiliated under PUMa's cover. Regarding your situation you might have to pay contributions.

You are working in France on secondment from a European company.
In order that your medical expenses are taken care of, you need to provide the following documents to the caisse d'assurance maladie in your area :
- form E 101 (A1) ;
- your European Health Insurance Card (EHIC) ;
- birth certificates for yourself and all members of your family ;
- copy of your passport or ID ;
- proof of residency (Utility Bills, rental contract, etc.) ;
- marriage certificate ;
- particulars of your bank account (RIB - relevé d'identité bancaire). These can be found readily available in your French cheque book.
You are living in a European country and your family is more or less permanently living in France.
So that they may receive reimbursement for medical expenses, please, provide us with :
- the Form application for the right to health insurance (S1106) (PDF) ;
- form E 109 (S1) in duplicate ;
- birth certificates for yourself and all members of your family ;
- passports copies ;
- proof of residency (Utility Bill, rental contract, etc.) ;
- marriage certificate ;
- particulars of your bank account (RIB - relevé d'identité bancaire). These can be found readily available in your French cheque book.
These documents will allow us to affiliate your family and take care of their medical expenses following the French legislation.
On the other hand, concerning your own charges, only expenses which are medically necessary will be reimbursed on presentation of your European Health Insurance Card.
You rent out chambres d’hôtes (bed and breakfast), you offer tables d’hôtes (evening meals), on a professional level. In other words the revenues from these activities make up your principal income.
You should make contact with your local Chambre de Commerce and contact SSI (Sécurité sociale pour les indépendants [Healthcare for entrepreneurs]) - Call 36 48.
From January 1st 2019, if you are starting this activity, you should make contact with your local Chambre de Commerce and you will also have to send the following documents to your local CPAM for your healthcare coverage :
- the printed Form application for the right to health insurance (S1106) (PDF) ;
- birth certificates for yourself and all members of your family ;
- marriage certificate ;
- passport or ID copies ;
- proof of residency (Utility Bills, rental contract, etc.) ;
- particulars of your bank account (RIB - relevé d'identité bancaire). These can be found readily available in your French cheque book.
If you are not a European citizen and you are not working in France, if you are a student older than 28 years old you have to apply to PUMa (protection universelle maladie).
You can make an application after 3 months of residency in France (legal residency requirements). Before that you have to be covered with a European Health Insurance Card (EHIC) or with a private insurance.
If you are entitled to be covered with PUMa, you will have to pay contributions to Urssaf (Union de recouvrement des cotisations de sécurité sociale et d'allocations familiales).
If you are a European citizen and you are not eligible to get a S1 form, you can't claim for the PUMa's benefits under 5 years of permanent residence unless your situation has changed while you've been living in France (change of income, divorce, long term disease, etc.).
